In Vitro Fertilization (IVF) in Madrid
In Vitro Fertilization (known by its acronym IVF) is the union of a woman’s egg and a man’s sperm in a laboratory. ‘In vitro’ means that it is beyond the body, outside of it. On the other hand, ‘fertilization’ means that the sperm has already fixed and has entered the egg, creating the embryo.

Phases of In Vitro Fertilization
In vitro fertilization treatment consists of different phases
1. Menstruation
2. Ovarian stimulation
In the natural process of a female menstrual cycle, women produce a single oocyte (ovum) each month. Through the administration of medication, what we seek in this first phase of In Vitro Fertilization is to stimulate the ovary and induce the production of more oocytes. If the creation of many oocytes is achieved, the possibility of gestation is greater since more embryos can be obtained. We monitor the evolution of this process through periodic ultrasound checks, and sometimes blood tests.
3. Embryo culture and in vitro fertilization
Once the oocytes have been extracted by puncture, and already with your partner’s sperm, the oocytes are fertilized in the laboratory using two techniques: conventional IVF or intracytoplasmic sperm injection (ICSI)
Post-embryo transfer process
About two weeks after the embryo transfer, the pregnancy test is performed. Two possibilities are contemplated after embryo transfer:
1. You get pregnant
The In Vitro Fertilization treatment has been successfully completed. From this moment, we discharge you and you continue with a gynecological monitoring of the pregnancy that can be, if you want, with our obstetric team, to continue with a personalized treatment.
- 2. You don’t get pregnant
When the cycle does not end with pregnancy, the most important thing is not to lose hope and stay calm. That you do not get pregnant in the first cycle of In Vitro Fertilization does not mean that you cannot do it in subsequent cycles. The two options before this possibility are:
a. If you have frozen embryos, we perform a cryotransfer, that is, the embryos that have been cryopreserved are transferred to your uterus as in the first cycle of IVF. It should be mentioned that preparing the uterus for a frozen embryo transfer is much easier, since it is not necessary to repeat the process of ovarian stimulation and follicular puncture.
b. If you do not have frozen embryos, we carry out a second attempt or second cycle, starting the In Vitro Fertilization process from scratch.
4. Follicular puncture
When it is verified by ultrasound means that the follicles have reached the appropriate diameter and number, the follicular puncture is programmed. Approximately 36 hours before the puncture, an injection of the hormone hCG should be administered, which induces oocyte maturation in a similar way to how it occurs in the natural cycle. The ovarian or follicular puncture consists of the extraction of the follicular fluid found in the oocytes by means of a surgical intervention that is carried out in the operating room, by means of sedation and whose duration does not usually exceed 15 minutes. Although it is not usual, if after the puncture you feel any discomfort, you can call the clinic to be informed of what steps to follow.
5. Embryo transfer
The embryo transfer is the last and decisive step in the process to achieve your pregnancy through In Vitro Fertilization. It is a crucial phase of treatment.
The process is very simple although, like the puncture, it is carried out in the operating room. The placement of the embryos is carried out with a specific catheter, made with a very flexible material, so as not to damage the walls of the uterine cavity, and you do not need to be sedated as it is not a painful process.
After the embryo transfer, you remain at rest for about half an hour. Then you can resume your daily activity, always in moderation. The embryos left over from the process will be frozen, in order not to repeat the process of ovarian stimulation and follicular puncture again, in the event that you do not get pregnant in the first cycle of in vitro fertilization.
6. Test to see if there is pregnancy
How many cycles can you do?
At Arpa Médica we will guide you in the most appropriate way to perform the following treatment. We will assess how the first cycle went and if the assessment is correct then we will advise you to repeat the In Vitro Fertilization cycle.
